I'm in Ohio; an "industry friendly state." Yes, I did. They wouldn't touch it. The fortune 500 company is 'self insured.' They blathered about - but bottom line, not touching it. I reconsidered some time later and was told by a different set that under Ohio law, you have one year to report or file (?). I reported while I was still out of my mind owing to the injury; yet the law might find me deficient regards reporting / filing. It's all documented; on my side of the equation anyway.
