Thank you! My training is that "Why" is sometimes not a helpful question, in that it can stir up a defensive answer, "Why did you do that?" But perhaps this is different, asking Why did it happen? I'm going to try!
Every time I ever submitted a 5 Why (ugh), the reviewer didn’t agree with my Whys. They’d change them arbitrarily and I’d just say “ok, will you sign off on it now?”
Yep! I’m right there with you as a retired Quality Nurse...love the process of a Root Cause Analysis and those pesky 5 why’s.
